Our program uses the domain management framework in routine clinical work as a holistic model to address the multidimensional needs and vulnerabilities of older adults with HFpEF and Cardiac Amyloidosis.
Patients are assessed using a variety of validated instruments for the following domains: medical, mind and emotion, physical function, social environment, and quality of life. These assessments are conducted on every new patient, as well as every 6 months following a patient's baseline visit to monitor changes.
Patient assessments are entered into and maintained in ClinvestiGator (https://ccim.weill.cornell.edu), a unique, online data collection system developed and based at Weill Cornell Medical Center.
